Patients are seen by appointment at any of our satellite clinics. Your primary care physician or specialist may refer you, but self-made appointments are always welcome. It is important to know whether or not your insurance provider will cover your visit, so please contact the appropriate billing specialist beforehand. Refer to the “Locations” tab at the top of the page to determine which clinic office is most conveniently located for you.

In compliance with HIPAA regulations, all medical records are kept strictly confidential and will not be disclosed to anyone without the patient’s written consent. If you wish to have records sent to another physician, we will request that you sign a release statement. If you have been referred by another physician, your records will normally be forwarded after treatment.
