The physicians at Spine Pain Diagnostics Associates participate with most insurance plans, including, but not limited to, plans such as Blue Cross Blue Shield of Michigan/Wisconsin (BSBC MI/WI), United Healthcare, Health EOS (WPPN, AHC, HCN, Multiplan), Network Health Plan, Humana Choicecare, PPOM, WEA, HealthSCOPE Benefits, GEHA, Workers Compensation, Medicare and Medicade.

In order to prevent any delays, it is very important to have accurate information. Please ensure you bring the following information to your appointment: insurance card and details and date of any of Worker's Comp Injuries, Motor Vehicle Accidents, and any other accidents.
